Objectives: To investigate the short-term effects of low-frequency repetitive transcranial magnetic stimulation (rTMS) over the motor cortex on writing parameters inn patients with writer's cramp. Design: Comparisons between writing performance before and after a 25-minute train of 1-Hz rTMS (1,500 stimuli) at 95% intensity of the motor threshold. Setting: Outpatient clinic at a department of rehabilitation medicine in a university hospital in Japan. Subjects: Five healthy subjects (mean age, 28 years, 4 males) and 5 patients with writer's cramp (mean age, 27 years, 4 males). Main outcome measures: Using a simple writing pressure gauge, maximum and average writing pressure ; degree of variation in writing pressure, which reflects the writer's skill in writing ; and writing time, which is the amount of time required. Results: After patients received rTMS all writing parameters improved, in particular, the degree of variation decreased with significance (p<0.01, three-way ANOVA). Conclusions: In writer's cramp suffers, low-frequency rTMS over the motor cortex can improve handwriting. The electronic pen is an easily used instrument that is helpful for analyzing writing patterns. (Jpn J Rehabil Med 2004; 41: 619-624) 要 旨: 書痙患者に対する低頻度反復経頭蓋磁気刺激 (rTMS) の有効性を, ペン型簡易筆 圧計を用いて検討した. 書痙患者5例 および健常群5名 に対して, rTMSを 一次運動野直上 に安静時運動閾値の95%の 刺激強度で1,500回 施行 した. 書痙患者では, 字体および書字評 価のすべての指標 (書字時間, 最大筆圧, 平均筆圧, 変動値) で改善がみられたが, 特に書痙 患者に特徴的な拙劣さの指標である変動値の改善が著しかつた. 一方, 健常群への影響は認め ず, 変動値において 「健常群 ・書痙群」「rTMS前 後」カテゴリー間の三元配置分散分析に有 意な交互作用 (p<0.01) を認めた. 本研究により, rTMSに よる書痙患者への効果が示さ れ, また, 簡易筆圧計による4つ の書字評価の指標を組み合わせることにより, より鋭敏に rTMSに よる治療効果をとらえることができる可能性が示唆された.
