Older people are much more likely to be victims of crime than perpetrators. Although aggression features in up to half of those with Alzheimer's disease, most aggression in the context of dementia is not serious and can be well managed by primary care and old age psychiatric services. Nonetheless, a small number of older people commit serious crimes. In the UK people over the age of 60 commit about 11 homicides and over 300 sexual offences each year. Many of the perpetrators are mentally disordered but we have little knowledge of them as there are no satisfactory studies of this group. In addition to those with mental health problems who offend for the first time in old age, a number of people, already in prison or secure hospital because of serious offences committed while younger, develop new mental health problems.
