In recent years, the use of communication robots for the purposes of supporting independence and reducing the care burden has been expected. Robot therapy has been confirmed to have the same effects on users as animal therapy, including physiological effects (eg improvement of vital signs), psychological effects (eg relaxation, increased motivation) and social effects (eg increased communication), and various effects are expected when robots act on human emotions. 1 Here, we report that the use of a baby-type robot (Smibi; Togo Seisakusyo, Aichi-gun, Japan) helped improve the healing process and reduce the care burden of an aged woman undergoing home-visit rehabilitation. The present patient was a right-handed woman aged in her 70s who had been diagnosed with right thalamic hemorrhage. Her anamnesis included normal pressure hydrocephalus, traumatic subarachnoid hemorrhage and hypertension. She required care for all activities of daily living. She lived with her husband, who was her primary caregiver. She underwent home-visit rehabilitation with a therapist twice a week for 60 min per session. Smibi was developed as a healing robot for older people. This robot laughs and cries like a baby when receiving care from a user. The present patient used Smibi for 1 h every day together with her husband for 1 month. During the home-visit rehabilitation, the therapist advised the woman how to use the robot and checked the status of its use. We carried out assessments based on Brunnstrom stage, handgrip strength, the Functional Independence Measure and the Zarit Caregiver Burden Interview at baseline and 1month. In addition, we interviewed the primary caregiver about the effects of Smibi. Table 1 shows the outcome measures of the case at baseline and 1 month. Zarit Caregiver Burden Interview scores improved from 38 to 34 points, and a reduction in the care burden was recognized in terms of care time and stress. No changes were seen in physical function or activities of daily living. In addition, the caregiver answered the interview as follows at 1 month: the case improved because of Smibi; her face became very expressive, her sitting time increased and she had more conversations with the caregiver.In recent years, many therapeutic effects of communication robots have been reported2, 3 however, to our knowledge, no studies using communication robots during home-visit rehabilitation have been reported. The objective of the present study was to examine the effects of Smibi on an older woman receiving homevisit rehabilitation. The use of Smibi contributed to the observation of a healing effect, a reduction in the care burden and the promotion of interaction between families. Furthermore, her sitting time increased and her bedridden time decreased, although no change was seen in her ability to carry out activities of daily living. These findings could lead to a better understanding of the mechanism underlying these interaction effects, and to the development of methods that allow these effects to be evaluated more objectively. As the present study was only carried out during a 1-month period, we plan to carry out a longer-term study in the future. In addition, we plan to increase the number of cases and evaluate cognitive function to confirm the utility of communication robots.
