Case Reports Case no. 1 A 37-year-old housewife presented with a 2-month history of perioral eczema mainly on the upper lip. The lips were oedematous and erythematous, with peripheral scaling outwith the vermilion border. The only relevant product that she reported was Fluorigard Gelkam, a toothpaste. 2 years earlier, she had been patch-test-positive to cinnamic aldehyde, Amerchol L-101 and cobalt. Patch tests were carried out to the current standard series, a dentists' series, face and cosmetics series and Gelkam. Despite previous positive results, these were all negative. Biopsy of the oral mucosa of the upper lip showed histological features of contact dermatitis. She undertook cinnamon-free, nickel-free and hypoallergenic diets, in turn, with no improvement. At review, she mentioned her long-term use of Boots Strawberry Lip Salve, with soothing extract of camomile. A patch test was positive. Patch tests to the individual constituents of the lipsalve were positive to the strawberry flavour ( 49392) I% pet., but not to the others (Table 1). Patch tests were subsequently carried out to the individual constituents of the strawberry flavour (Table 2), and a positive reaction was observed to pure maltol I% pet.: 30 controls were negative to the same preparation. The use of the lipsalve was stopped and the cheilitis resolved within a few days. It has not recurred in the following 12 months.
