Seventy-one patients who underwent augmentation mammaplasty were examined one to five years after surgery. Thirty-seven of these patients, chosen at random, were interviewed by a psychiatrist. The follow-up showcd that quite a few patients had complaints about their implants. No less than 59% of the patients complained about the implants being too hard, causing practical problems. Fibrous envelope contractures around the breast implants caused irregularities of the smooth implant surface. In 7% of the patients, dislocation vvas severe and difficult to hide. Additional problems occurred such as lost or reduced sensation in the nipples and unsatisfying scars.Three persons (one nurse, one aide, one photographer) independently judged the cosmetic results according to a scale of “very good,"" good,"“acceptable," or" bad." The surgeon did the same classification, and opposite these judgments the patient's subjective opinion was placed in the scale “very satisfied,"" satisfied,”“doubtful," or “dissatisfied." Objectively the results were considered very good in 13 to 49% and bad in 8 to 13%, while the patients themselves were very satisfied in 84% and dissatisfied in only 5%, this in spite of the subjective complaints about the implants. These results are no doubt duc to psychological factors. Preoperatively the patients feel their feminity inadequate and have low self-esteem, contributing to a tendency toward isolation and passivity. Some social situations grow to vast problcms. Scnsitivity reactions occur and also different degrees of insecurity in the sex life. These problems often result in a depressive reaction that sometimes can end in suicide.
