Most patients who have cardiac arrhythmias have heart disease and nonarrhythmic dysfunction, for example, ventricular failure, angina pectoris, or hypertension. Patients who take antiarrhythmic drugs are very likely to take other cardiovascular drugs to treat these coexistent problems. Also, because of age and other factors, patients taking antiarrhythmic drugs are likely to take one or more noncardiovascular drugs as well. Finally, it is notable that the tolerance for a drug interaction may be quite low in patients who take cardiovascular drugs. For example, postural hypotension caused by a drug interaction might be well tolerated in a normal young person. Patients with poor left ventricular reserve or with critical stenosis of coronary or carotid arteries, however, tolerate postural hypotension poorly. In this paper, we will discuss adverse drug interactions involving antiarrhythmic drugs.
