have obscured positive findings on the quantitative measures favoring tacrine. Because a greater percentage of patients on lower tacrine doses were institutionalized, and hence more severely affected, the exclusion of these patients from follow-up assessments would underestimate the effect of higher doses of tacrine on the quantitative end points. On the other hand, if there were truly no benefits from tacrine after 2 years on these quantitative measures, the uncertainty created by attrition weakens the claim of no benefits also. Because of disease decline over 2 years, and subsequent attrition, mental status examinations or functional assessments used in traditional 6-month clinical trials in patients with mild to moderate disease severity are inadequate as outcome measures in long-term clinical trials. Outcome measurement techniques that use survival analyses and instruments with broader ranges are preferable for long-duration anti-AD drug trials. The recently published controlled trial of selegiline, a-tocopherol, and placebo? is an example of another application of this methodology. As was seen in our study,’ there were benefits in this latter study3 in terms of delays in primary end points and activities of daily living, but no benefits in terms of cognitive test scores.
