In 2 cases with a puzzling mixture of clinical pulmonary and renal manifestations, autopsies revealed a complex of granulomatosis with angiitis of the lungs, massive necrosis of the spleen and focal thrombotic granulomatous glomerulonephritis. In addition, in 1 of the cases, focal necrotizing granulomas were seen in many other organs, and lesions similar to those of periarteritis nodosa were found in the gastrointestinal and genitourinary tracts. In this article, the term necrotizing granulomatosis will be used, for the sake of brevity, to include the entire complex. A search through the literature revealed a number of reports of necrotizing granulomatosis often with varied interpretations and diagnoses, suggesting that the protean manifestations have meant many things to many investigators. The publication of these 2 cases is prompted by the desire to clarify somewhat the confused situation caused by the varied interpretations as well as by the interest aroused in a lesion, description of which has appeared with increasing frequency in the literature. In a subsequent publication the possible relationship of pulmonary necrotizing granulomatosis with angiitis to atypical pneumonitis, particularly of the cholesterol type, will be discussed.
