In patients with advanced cancer, a readiness to address pain and other intolerable symptoms is a medical and moral imperative. Systemic analgesic therapy in accordance with the World Health Organization (WHO) analgesic ladder, combined with primary antitumor treatment, provides adequate relief for up to 70–90% of patients [1]. The management of the 10–30% of patients who do not achieve adequate relief with systemic therapy is challenging and it requires a familiarity with pain assessment, physiology and a range of therapeutic strategies.
