A new method for the consistent production of endocarditis in dogs is described. Aortic insufficiency was produced in 15 dogs by perforating the base of one or both coronary cusps of the aortic valve. Beginning 8-159 days after surgery, the dogs received intravenous injections of broth cultures of either S. aureus or S. mitis JH26 (Lancefield D). The clinical course and pathologic picture resembled those of acute and subacute bacterial endocarditis, respectively. All 15 dogs developed valvular lesions, confined largely to the contacting surfaces of the aortic and mitral leaflets and contiguous areas. Dogs given S. mitis often developed an acute diffuse proliferative glomerulonephritis and focal glomerular lesions. Ten control dogs, including 2 with sham operations, were similarly injected and showed no such lesions. An increased cardiac work load and perhaps a relatively diminished coronary blood flow are considered possible important factors contributing to the increased susceptibility to endocarditis.
