We prove a structural theorem that provides a precise local picture of how a sequence of closed embedded minimal surfaces with uniformly bounded index on a given three-manifold might degenerate. This leads to a surgery result to cut out these degenerating regions, yielding several compactness-type theorems for such surfaces.
