The percent free testosterone (T) and estradiol (E2, the percent T and E2 bound by T-E2-binding globulin (TeBG), and the T-binding capacity of TeBG in the daily plasma of five normal menstrual cycles were measured. These binding parameters tend to remain constant throughout the cycle. Their respective mean (±SE) values were: 2.61±0.04%, 54.5±0.82%, and 22.2±0.69 ng/ml. The daily plasma concentrations of the free, the nqn-TeBG-bound (index), and the total T tend to parallel each other throughout the cycle; they were elevated at the mldcycle and dec; reased during the luteal phase. The daily E2/T ratios of the percent'free and the percent TeBG-bound fractions remained constant throughout the cycle, while the daily E2/T ratios of the free, index, and total hormone concentration showed a pattern similar to that of the daily E2 level in the menstrual cycle. In a hyperandrogenic state, such as in hirsute women or men, the decreased E2/T ratio was associated with (1) increased percent free T and E2,(2) decreasec; l percent TeBG-bound T and E2, and (3) decreased T-binding capacity of TeBG. In a hyperestrogenic state, such as in pregnant women, the elevated E2 tends to (1) decrease the percent free T and E2,(2) increase the percent TeBG-bound T and E2, and (3) also drastically increase T-binding. capacity of TeBG. Changes in the binding parameters of E2 and T were not detected until the E2/T ratio fell outside the wide normal range.
