Sera from 147 polytransfused patients with homozygous thalas saemia were investigated for the presence of antibodies against red cell antigens; 79 sera were tested for platelet antibodies; 60 of the above samples were also investigated for the presence of antibodies against white cells, Gm, Inv and Ag antigenic determinants. The frequency and specificity of each type of antibody were correlated with the amount of transfused blood, transfusion reactions and clinical parameters, such as splenectomy. The patients, from 3-to 24-years-old, had received from 10 to 287 transfusions over a period ranging from 2 to 18 years; pre-transfusion blood samples were collected from all cases. The presence of red cell antibodies was investigated using agglutina tion in saline and anti-globulin tests [1]. Anti-K antibodies were de tected in 3 out of 147 patients; a further incomplete warm antibody of unidentified specificity was detected in another patient. Moreover, 4 Rh-negative children out of 9 developed antibodies against Rhantigens after an unknown number of incompatible blood trans fusions; all 4 patients formed anti-D; one also had anti-C and another anti-C and anti-E. Two patients had anti-I cold agglutinin (titre 1: 8). Leucoagglutinins were detected by the EDTA agglutination tech nique [21], against a panel formed by 5-8 donors. Cytotoxic anti bodies against lymphocytes were detected by the microdroplet tech-
