To the Editor: Merkel cell carcinoma (MCC) is a rare neuroendocrine skin tumor with a high risk 52 of local and distant spread. Despite the increase in sentinel lymph node biopsy (SLNB) 53 utilization as a result of recent NCCN recommendations, there remains a paucity of data 54 regarding outcomes in clinically localized MCC patients with complete pathologic nodal 55 microstaging. 1 In MCC, disease relapse is a poor prognostic sign and efforts to prevent 56 recurrences may result in improved survival. 2 This multi-center study was designed to examine 57 patient, tumor, and treatment characteristics among pathologic stage I and II MCC patients and 58 characterize patterns of recurrence and patient survival. These data can help inform surveillance 59 strategies for localized MCC. 6061 Adult patients from six centers with American Joint Committee on Cancer (AJCC) 8th edition 62 pathologic stage I-II MCC from 2005 to 2020 were identified. Study inclusion required patients to 63 undergo resection with SLNB that was negative for micrometastatic disease. Various patient, 64 tumor, and treatment characteristics were considered in the analysis and compared in stage I-II 65 patients (Supplemental Table 1). Univariate analysis for categorical and continuous variables 66 was performed using Pearson’s χ² test and Wilcoxon rank-sum test, respectively. Predictors of 67 all-site recurrence were determined based on Cox proportional hazard regression analyses; 68 disease free survival (DSS) was analyzed using the Kaplan-Meier method and compared using 69 the stratified log-rank test. 70
