A 64-year-old woman was admitted to our hospital with breathlessness on exertion. She has experienced central cyanosis for more than 50years. Home oxygen therapy (HOT) was introduced for hypoxia 7 years ago. She has been suffering from frequent epistaxis of unknown cause.On admission, she had clubbing nails and telangiectasias of the buccal mucosa and nail bed. Chest roentgenography in a posterioanterior projection showed a large snowman-like shadow in the hilum of right lung (Fig. 1A). Volume-rendering multi-detector computed tomography revealed that the shadow was a pulmonary arterio-venous malformation (AVM) forming giant aneurysms (Fig. 1B).
