The clinical records of 506 patients with Hashimoto''s thyroiditis seen at the Mayo Clinic are reviewed. Of these, 119 patients (23.5%) had coexistent musculoskeletal diseases or symptoms. Among a group of 243 patients with granulomatous thyroiditis, only 3 (1.2%) manifested coexistent connective-tissue disorders or symptoms. 4% of the patients who had Hashimoto''s thyroiditis also had rheumatoid arthritis, a tenfold increase in the incidence of rheumatoid arthritis as compared with acute granulomatous thyroiditis. In addition 7.9% had a fibrositic syndrome, and 10.7% had other unexplained musculoskeletal symptoms. An unusual chest-pain syndrome was found among 11.9%. In a significant percentage of the patients the secondary fibrositis or chest pain could not be attributed to any process other than Hashimoto''s thyroiditis. Other patients had rheumatoid spondylitis, possible lupus erythematosus, the hydralazine syndrome, SjOgren''s syndrome and scleroderma. The capacity to develop an autoimmune reaction may represent a feature common to patients with Hashimoto''s thyroiditis and certain of the collagen diseases.
