A57-year-oldwhitemanwithhypertension,hypercholester-olemia, and a 12-year history of type 2 diabetes mellitusreturned for follow-up. He was found to have microalbumin-uria 4 years ago, presumed secondary to diabetes. He pro-gressed to stage 2 chronic kidney disease (CKD) 2 years ago.He feels well except for 2 episodes of hypoglycemia in the lastmonth. He denies symptoms of neuropathy, and a dilated-eyeexamination 3 months ago did not show retinopathy. He isadministered simvastatin, 20 mg/d; lisinopril, 20 mg/d; met-formin, 1,000 mg twice daily; and glyburide, 5 mg twice daily.On examination, his body mass index is 27 kg/m
