SIR. Most patients with bullous pemphigoid (BP) have high serum levels of IgR' '' which appear to be related to disease activity. The skin structure at which they are directed is unknown. In order to investigate the possible role of IgK antibodies in BP. we studied nine patients (seven male, two female) with typical clinical and histopathological features of BP. None had a history of atopy, nor were they taking immunosuppressive therapy. Three atopic patients with high serum IgK levels (587-43f) ll/nil; normal value < 100) as determined by radioimmunoassay (RIA) and two subjects with low serum IgE levels served as controls. Direct and indirect immunofluorescence (IF) were performed by standard methods using Meloy GAM IgF FITC and Kallestad GAII IgC, IgA. IgM.C3. C4 FITC. Human skin, both entire, and split at tbe lamina ludda level with 1 M NaCI. and monkey oesophagus, served as substrates for indirect IF. The serum IgE levels were determined by RIA. On direct IF all patients had deposits of IgG and C3 at the basement membrane zone (HM7) in a linear pattern typical of BP or epidermolysis bullosa acquisita (F.BA). Two patients also had very slight discontinuous IgK deposits at the BMZ. In I'atieiU 4 the deposits were granidar and in Patient T linear. Six patients had IgG antibodies in their serum directed at the BMZ. using monkey oesophagus as a substrate. The antibodies bound to the roof of I M NaCl-induced splits in five patients (Fig. 1) and to both the roof and the base in one. The titres of anti-BM7 IgG ranged from 1/40 lo l/(i40. Seven patients had •serum IgM levels grt-att-r than 1 (10 U/ml (1 50112 3 U/ml) that were apparently unrelated to the extent of the disease. None of ihe controls had circulating antib{Kiies to BMZ. Anti-RMZ IgF. antibodies were detected in six patients using monkey oesophagus as substrate, with titres ranging between 1/40 and 1/160. They hound to the roof of the bulla in human split skin in ail cases (Fig. 2). None of the controls had anti-RMZ [gi; using monkey oesophagus as a substrate.
