Essential fatty acid deficiency, although well documented in animals, has been produced in human beings only through severely restricted oral diets. Recently, evidence for this deficiency state has been demonstrated during prolonged fat-free parenteral alimentation. The present case report demonstrates a case of severe essential fatty acid deficiency which occurred during a period of fat-free parenteral alimentation and in which the scaly skin lesions, growth retardation, and altered plasma fatty acid pattern seen previously in infants receiving oral diets deficient in essential fatty acids were observed as well as the thrombocytopenia previously reported in an adult on prolonged fat-free parenteral nutrition. Intralipid was found to be clinically effective in correcting this deficiency in a patient tolerating only parenteral alimentation.
