An assessment of what the future holds for cardiac positron emission tomography (PET) is, at best, challenging. Often the past is a prelude to the future. Yet the current rapid and unprecedented changes in the medical enterprise might limit extrapolations from the past. The forces at work might profoundly alter the delivery of health care in both the private and the public sectors. Indeed, they might usher in the" industrialization" of medicine. Similar forces affect the research environment as a critical element for the initiation, development, and maturation of new diagnostic and therapeutic approaches. At the same time, changes foster opportunities. In evaluating the future clinical role of cardiac PET, it is important to enumerate its main advantages and its limitations. Among its advantages are the availability of a virtually unlimited number of radiotracers and the high temporal and spatial resolution. Both are key ingredients for its ability to provide measurements of regional functional processes in absolute units. Limitations include technical and logistic complexities and the current high start-up and operational cost. Both advantages and limitations will undoubtedly affect the clinical future of PET.
