Treatment nonadherence is a substantial problem in bipolar disorder treatment, often associated with mood relapse, hospitalization, and even suicide. 1, 2 Individuals with the rapid-cycling variant of bipolar disorder (RCBD) appear to account for 16% of patients who have bipolar disorder; 3 however, treatment adherence has not been well-studied among individuals with rapid cycling. This is a secondary analysis from a larger treatment study (MH R21 62650) examining treatment adherence among individuals with RCBD participating in a clinical drug trial. Individuals who dropped out of study for nonadherence were compared with individuals who were adherent to medication.All participants were enrolled in either of 2 ongoing randomized, double-blind trials evaluating the efficacy and safety of triple-drug therapy (lithium, valproate, and lamotrigine) compared with double-drug therapy (lithium and valproate) for the treatment of type I or II RCBD. The 2 studies were identical in design except that 1 trial focused on individuals with dual diagnosis (RCBD and substance abuse/dependence), whereas the other study focused on individuals with RCBD alone.
