Individuals who live at high elevation are subject to reduced atmospheric oxygen (O2) tension (PO2), leading to decreased alveolar PO2, reduced arterial PO2, and limited O2 delivery to tissues. In some cases, maladaptive compensation for chronic exposure to high altitude leads to chronic mountain syndrome (CMS) even for individuals who are native to high altitudes. Limited arterial O2 saturation and hypoxia drive the erythropoietic system to excessive erythrocytosis, resulting in increased total haemoglobin (Hb), red blood cell (RBC) count, and haematocrit (Hct). Individuals with CMS can also develop pulmonary hypertension, sleep disturbances, headaches, tinnitus, and other clinical symptoms. Most of the studies until now have suggested that the clinical symptoms of CMS stem from the increase in blood viscosity resulting from increased Hct, but evidence presented in a study published in this issue of The Journal of Physiology by Stauffer et al., titled Blood viscosity and its determinants in the highest city in the world shows that high altitude polycythaemia alone does not sufficiently explain the haemorheological changes occurring in CMS individuals
