Autologous keratinocyte grafting is used for treating poorly healing wounds such as chronic venous leg ulcers. 1 Starting with a small skin sample from the patient, basal keratinocytes capable of proliferating are isolated and then expanded in tissue culture usually by the method of Rheinwald and Green. 2 Co-culture with growth-inhibited 3T3 feeder cells and complex medium containing fetal calf serum (FCS) enriched with growth supplements is essential for sustained keratinocyte proliferation. 3 Once a sufficient number of keratinocytes has been obtained, graft preparations can be prepared. Products derived from non-autologous serum, and FCS in particular, carry the risk of transmitting infectious agents such as viruses and prions. The benefits, however, of autologous or allogenic keratinocyte grafting seem to outweigh the risks of infection as regulatory authorities have raised little concern over this issue.
