Citro and colleagues newly issued the article entitled “Long-term outcome in patients with takotsubo syndrome presenting with severely reduced left ventricular ejection fraction” 1. This is a fascinating article to assess the long-term prognosis of takotsubo syndrome (TS) patients with reduced left ventricular ejection fraction (LVEF). LVEF less than 35% on admission was the prognostic indicator not only in the acute phase but also in the long-term follow-up period in TS patients. It is thought that various studies have already presented enough clinical data in TS, although, the data of this study are presenting directly associated hints and tips in TS prognosis. TS is previously thought to be a benign condition not requiring any specific treatments; however, nowadays the prognosis of TS is regarded not optimistic than before due to several complications and life-threatening cases. El-Battrawy et al. demonstrated a prognostic indicator in TS patients 2 and concluded that LVEF less than 35% was the independent predictor of short-and long-term mortality in TS patients. Citro et al. reported the results of 326 patients with TS from the Italian multicenter registry data that included approximately 3 times as much as the previous published data. Recently, the RETAKO registry promoted by a Spanish researcher group reported that cardiogenic shock was associated with short-and long-term prognosis in patients with TS 3. The decreased systolic blood pressure on admission is thought to be a prognostic indicator in TS patients 4; thus, these facts support the risk stratification which has been previously proposed and described in the position statement 5.
