MATERIALS AND METHODSThis study includes 18 heart transplant patients operated at our institution between December 1998 and December 1999,(namely 15 men, 3 women, mean age 51 years). The mean donor age was 38 years. All patients received rabbit anti-thymocyte globulin as induction therapy. Cyclosporine, prednisone, and imuran were given for maintenance immunosuppression. Cyclosporine was initiated after the operation with the dose being adjusted according to renal function. Two biopsies specimens per patient were studied for a total of 36 biopsies. In nine patients the first biopsy was done during the first post transplant week followed by a second biopsy 2 months later. For the other nine patients, the biopsy that was selected occured during an episode of acute rejection; a second biopsy was chosen after the rejection period. The diagnosis and grading of acute rejection episodes was performed using light microscopy according to the criteria standardized by the International Society of Heart and Lung Transplantation; five biopsies showed grade IA rejection and three, grade IB rejection.
