An account of the development, growth and subsequent rupture of intracranial aneurysms based upon the dissection of 163 ruptured and 40 unruptured cerebral aneurysms is presented. The aneurysms develop from one of the arterial junctions of the circle of Willis, are multiple in nature in 1/8 of the cases and may wrap around adjacent vessels and nerves and/or become bilocular as they grow. At a diameter of 6-15 mm they may produce clinical effects, some simulating tumors. Rupture occurs at the fundus of the aneurysm in at least 64% of the cases. Cerebral arteries are more prone to aneurysms due to a thinner media and a poorly developed to absent external elastic lamina. Other factors related to their occurrence are developmental faults in the media, blood pressure changes and atherosclerosis.
