Establishment of pancreatic intraepithelial neoplasia (PanIN) as a putative precursor lesion of pancreatic carcinoma has allowed analyses of the molecular changes associated with stepwise progression of pancreatic carcinogenesis. 1 Pancreatic intraepithelial neoplasias are divided into 4 categories, PanIN-1A, PanIN-1B, PanIN-2, and PanIN-3, according to morphological features. Several studies, including polymerase chain reaction and immunohistochemical (IHC) analyses, of resected pancreatic carcinoma specimens have been performed. It was recently reported that several molecules are involved in PanIN progression. 2When PanIN is found near an invasive carcinoma, it is difficult to differentiate completely cancerization of the duct from primary invasive carcinoma. Takaori et al 3 reported that cancerization of the duct (intraductal growth from invasive carcinoma) occurs when nearby normal ductal epithelium is abruptly replaced by dysplastic epithelium. We previously reported that some pancreatic ducts show gradual transition from early PanIN to PanIN-3, suggesting that pancreatic carcinoma arises from stepwise progression of PanIN. 4 Furthermore, in previous studies of PanINs, researchers defined a single-grade PanIN (PanIN-1A, PanIN-1B, PanIN-2, or PanIN-3) as a single duct, and results were compared between grades. 5 When we examined the histopathologic data of previous studies, we noticed a single duct showing gradual transition from low-to high-grade PanIN. We termed this lesion as a gradually transitional component (GTC). 4, 6 Immunohistochemical analyses of GTCs are helpful to understand changes in expression of various molecules during progression of PanIN, which may clarify the molecular mechanism that underlies PanIN progression.
