A 63-year-old woman fell and sustained a rib fracture 2 years ago, fractures of the lumbar spine the next year and had difficulty walking. She was referred to our department with raised serum alkaline phosphatase and hypophosphatemia and a complaint of systemic pain.Her height and body weight were 152 cm and 49 kg, respectively. Her complete blood count, liver function and renal function were in the normal range. Serum calcium was mildly decreased [8.7 mg/dl (8.8–10.1)], phosphate was extremely low [1.1 mg/dl (2.4–4.3)], tubular phosphorus reabsorption threshold
