DiscussionIPV is normally used to deliver a second twin with the membranes still intact (Cunningham et al. Citation 2014). Neglected shoulder presentation is a rare foetal malpresentation in which the foetal shoulder is firmly packed into the pelvis that can occur after membrane rupture in cases of transverse lie. In this situation, especially if the baby is alive, caesarean section is recommended, given the relatively high risk of uterine rupture due to uncontrollable uterine contractions (Mahajan et al. Citation 2009; Okonofua Citation 2009). However, the management of neglected shoulder presentation in cases of IUFD remains controversial. Here we report the first case of neglected shoulder presentation with IUFD due to placental abruption in which IPV and breech extraction were successfully performed after general anaesthesia and intravenous nitroglycerine was administered to the mother.
