Recently we reported a case of contact dermatitis to garlic. This case lead us to include garlic extracts in the routine patch test series. Tests with aqueous and ethanol extracts of garlic were performed in 63 patients with ulcera cruris and in 168 persons with contact dermatitis.In 6% of them, mostly in women over 40 years old, positive reactions were observed. Onion extracts did not give rise to positive tests, and no crossreactivity could be observed between garlic and onion. In only 1 out of 8 patients with positive tests to garlic a tulip extract elicited visible skin responses.The active component was found susceptible to heat, and to acid treatment.Gaschromatographic analyses indicated the presence in garlic extracts of tuliposide-B. Tuliposide-A (the allergenic component of tulip bulbs) could not be demonstrated. Tuliposides (A and B) are to all probability not responsible for the skin responses observed after epicutaneous challenge with garlic extracts.
