项目摘要

令和4年度は、コロナ禍を経て約3年ぶりにラオスでの調査を再開することができた。2022年4-5月・7月に調査ビザの手続きを、2022年8-9月及び2023年2月に定量的な身体活動・食事調査と健康状態に関する聞き取り調査を実施し、以下のような知見が得られた。◆サトウキビ契約栽培の導入による性的分業の変化：NN村は2021年にサトウキビの契約栽培を始めた。2019年8-9月 (雨期) の主な生業活動は、男女ともに焼畑・水田の除草だったが、2022年の雨期では顕著な性的分業（女性：除草、男性：サトウキビの収穫に使う竹ひも作り）が確認された。2023年2月 (乾季) は、男女ともにサトウキビの収穫が主な仕事であった。サトウキビ契約栽培の導入によって、特に女性の身体活動量が増加し、数kgの体重減少がみられた女性もいた。◆加工食品の消費量の増加：サトウキビ栽培で多忙になり、野生動植物をとる時間がなくなったことで、加工食品 (インスタントラーメンや魚の缶詰) の消費量が増加した。食事の内容に顕著な男女差は認められなかった。◆避妊注射の影響：聞き取り調査によって、避妊注射が女性の肥満の一因である可能性が浮かび上がった。たしかに過体重・肥満だった女性のうち、1人を除く全員が避妊注射を受けていた。避妊注射は無料で受けることができるが、点滴やピルの料金は自己負担である。それでも太りたくないという理由で避妊注射を避け、点滴やピルを利用している女性もいた。以上のように、NN村における肥満の男女間差は性的分業と食事からは説明が難しく、避妊注射に含まれる性ホルモンが女性のエネルギー代謝に関係している可能性が示唆された。詳細についてはさらなる研究が必要であるが、注射以外の避妊法も無料で提供するなど医療サービスを拡充させることで、ラオス農村における肥満リスクの男女差が是正される可能性がある。
