项目摘要

本研究の目的は、認知症者の終末期における意思決定を支援するツール・スキル・マネジメント・教材の作成と評価である。令和４年の研究目的は、①令和３年に作成した２つの教材を使って、多職種で支える意思決定支援の育成を行い、評価すること。②死亡前30日間の症状マネジメントと医療行為、意思決定支援の関連を明らかにすることである。①については、意思決定支援のツールであるACP（アドバンス・ケア・プランニング）を主題にした教材（ケース教材）を使って、多職種参加型ディスカッションを２回実施し、内容を質的に分析した。その結果、ACPは有効な意思決定支援ツールではあるが、運用に当たっては「ACPの理念がメンバーに浸透しない」「ACPを運用することが目的化してしまい、理念が形骸化しやすい」など運用面での課題が抽出された。②については、高齢者施設における死亡前30日間の症状や変化について文献レビューを行った。国内外の15文献をレビューした結果、認知症高齢者の終末期は穏やかな経過をたどるため、症状の変化をはっきりと把握することが困難であるが、死亡前１週間は、「傾眠状態」「食事嚥下機能の低下」「体重減少」「血圧の低下」などの症状が確認できた。これらの症状に対する医療行為は「経過観察」「安楽な体位の工夫」「食事形態の変更」など症状緩和に関するものが多かった。これらの結果は、認知症者に限定したものではなく、高齢者の終末期に共通する結果であることを確認できた。次年度以降は意思決定支援との関連を明らかにするために、質問紙調査（予備的調査を含む）を実施したい。
