项目摘要

今年度は、システマティックレビューおよび実態調査（質問紙およびインタビュー調査）を実施した。システマティックレビューは、医中誌WebおよびCINiiで検索した国内の14文献を対象に、認定特定行為業務従事者等の介護職による喀痰吸引等の現状と課題に関する記述の計量テキスト分析を行った。結果、介護職による喀痰吸引等の現状および課題として、①医療的知識や技術・経験不足から介護職は喀痰吸引等および急変時対応において不安がある、②介護職の業務量増加と責任追加による身体的・心理的負担の増加に伴うストレスがある、③現場における喀痰吸引等の指導内容とその方法に余地があるが抽出された。また、介護職による「利用者の全身状態のアセスメント」は、現行の介護職の基礎教育内容や喀痰吸引等研修では困難を伴うことが明らかになった。しかし、「喀痰吸引等業務計画書」の視覚化等の工夫により、介護職による観察や報告の正確性を高める可能性が示唆された。また、「認定特定行為業務従事者」である介護職および指導にあたる看護職を対象にした質問紙調査から、介護職の喀痰吸引等の実施には、両職種ともに肯定的であった。しかし、吸引の実施頻度が少ない介護職は「喀痰吸引等の実施をやめたい」思いが強くなる傾向があることから、実際に吸引等手技を継続して提供できる環境作りの重要性が示唆された。また、安全な喀痰吸引等の実施に向けて介護職は、「ヒヤリハット等事例」に関するフォローアップ研修を希望していたことから、ヒヤリハットに対応するシミュレーション・プロトコール等の開発の必要性が高いことが明らかになった。1回目のアンケート回収率が低かったため、追加調査を行ったが結果分析まで至っていない。次年度は、質問紙調査（追加分）とインタビュー調査の分析を行ったうえで、シミュレーション教育プログラムの開発及び評価を行う予定である。
