项目摘要

全国4000病院を対象としたアンケート調査およびインタビュー調査のデータの分析を実施した。分析した結果、日本の多くの病院で身寄りがない人の入院を受け入れている現状が明らかとなった。身寄りがない人の入院の困りごととしては、医療の決定が高い割合を占めていた。また、身寄りがなく意思決定が困難な人の医療の決定プロセスとしては、医療チームでの決定が高い割合を占めていた。この分析結果を学会発表した。国内外の先行研究をさらに集めてレビューを実施し、日本における身寄りがない人の実態について論文としてまとめた。論文は受理され、今年度中にオープンアクセスジャーナルに掲載予定である。この分析に基づき「身寄りがない人の入院及び医療に係る意思決定が困難な人への支援に関するガイドラインに基づく事例集」の作成に協力をした。事例集は、身寄りがない人の入院に関する法的懸念事項、倫理的懸念事項を整理し、対応案を示したものである。本研究目的である医療・ケアチームによる医療の決定プロセスの標準的な方法を提示することができた。事例集については研修会やセミナーを通して周知に努めた。
