项目摘要

本研究では、COVID-19による外出自粛や行動制限が糖尿病患者の受診行動や予後に与えた影響を検討するため、健康保険組合由来のレセプトデータや医療機関由来の診療録データを用いて、糖尿病患者の受診控えの頻度や経年変化を定量的に記述し、血糖コントロールや合併症発症との関連を検討することを目的としている。初年度の2021年度は、研究計画書を京都大学医学研究科・医の倫理委員会へ提出し、研究実施の承認を得たうえで、データ提供会社へのデータ抽出依頼を実施した。また、本研究に関連した研究として、学外の共同研究者よりJACSIS調査データの提供を受け、COVID-19流行下における疾患別の受診控えと疾患悪化との関連を記述し、糖尿病患者では他疾患患者に比べて、自己申告で1.5倍程度の病状悪化の傾向があることを明らかにし、英文国際誌に報告した（Family Practice. 2022. 24;39:883-890）.2022年度は、COVID-19と受診控えに関する文献調査を行いつつ、本研究のレセプトデータおよび診療録データの分析を開始したが、データ対象期間が限定的であったため、2022年末にデータ提供の再依頼を実施し、2023年1月に新たなデータセットを得て解析を再開した。また、学校健診情報を用いてCOVID-19流行期の学童の肥満増加の検討を行い、成人の糖尿病との関連についても検討を加えながら、COVID-19流行による行動制限の健康への影響について考察を深めている。

This study aims to investigate the impact of COVID-19 on the behavior of diabetic patients, the origin of health care, the use of medical records, the frequency and aging of diabetic patients, and the relationship between diabetic complications.
