本研究は，肝外胆管癌における癌細胞の胃腸分化マーカーおよび癌間質における癌関連線維芽細胞 (cancer-associated fibroblasts, CAFs)，腫瘍浸潤リンパ球(tumor-infiltrating lymphocytes, TILs)，腫瘍関連マクロファージ (tumor-associated macrophages, TAMs)マーカーの発現状態を調べ，肝外胆管癌の癌細胞と癌間質の関係を明らかにすることを目的としている．加えて，肝外胆管癌の予後を規定する癌細胞と癌間質マーカーの組み合わせを解明することについても検討している．なお，本研究の予備実験も含め，我々の施設で明らかにした膵胆道系の癌の診断に有用と考えられる癌の分化に関連するマーカーについても検討を開始した．研究実施計画に沿って研究を進めており，胆管癌手術施行例の集積，HE標本全ての見直しを行い，主座，組織型，深達度，脈管侵襲，上皮内癌の有無，マーカー発現などの病理学的所見を評価した．管状腺癌において特に癌先進部における上皮胞巣の大きさ，形態，胃腸分化マーカー発現の特徴，周囲間質の形態，マーカー発現の特徴について評価を行っている．当初は最も深部まで浸潤し十分な腫瘍量が含まれる代表切片を選択し，評価対象となる部位を絞りtissue arrayを作製した上で評価する予定であったが，予定を変更し代表切片のブロックそのものを用いて検討している．予後の症例の集積は順調であり，予後データと合わせて統計解析も随時行っていく予定である．

In the beginning, the depth of the tumor was very high, and the content of the tumor represented the selection of the slice. For example, the location of the disease is known as the tissue array, and the predetermined temperature represents the use of the slice, which represents the section of the slice. in the future, the collection of cases of post-morbid disease is not valid, and the statistical analysis of the statistical data will be used at the same time.
