悪性中皮腫（malignant mesothelioma: MM）は化学療法等治療抵抗性の極めて予後不良（診断後生存中央値約１年）の腫瘍で、世界中で今後さらに患者数が増えることが見込まれる。その変異は、塩基配列レベル変異は小児がんレベルと少なく、技術的に捉えにくい微小欠失が複数個所で起こり、激しいゲノム再構成（chromothripsis-like patterns：CTLP）が生じている。CTLPが悪性化との関連性や、免疫チェックポイント阻害剤の有効性に寄与する可能性があるため、エクソン単位のゲノムコピー数（CN）変化を精度高く解析可能なMLPA（Multiplex ligation-dependent probe amplification）と次世代シークエンス解析を組合せた新規手法digital MLPA を開発している。網羅的にゲノム再構成を検出できるようにMM 用に改良し、digital MLPA とターゲットシーケンスによりMM の変異を捉えている。特に、予後良好と不良患者のゲノム変化を比較することで、早期診断マーカーや予後関連因子を探索することが目標である。同法を用いた腎細胞がんのCN解析では、患者予後悪化とCN変化数の増加には相関が見られ、１年内に転移が生じた予後不良患者由来の腫瘍でCDKN2A遺伝子に両アレル欠損が検出された。しかし、MMでは５年以上生存の予後良好者でもBAP１遺伝子周辺にCTLPが検出され、意外にも本領域のCTLPは予後と関連しないようだった。CDKN2A遺伝子の両アレル欠損頻度は予後不良者より低いものの検出され、CN変化の個人差が大きい。さらに解析検体数を増し、MM 患者の早期発見・再発モニタリングを実現可能な分子マーカーを同定し、目標を達成する。その過程でゲノム不安定性を誘発する標的分子が見つかれば、将来の治療法開発につなげる。

Malign mesothelioma (MM) is extremely resistant to chemotherapy and other treatments and has a poor prognosis (median survival after diagnosis is about 1 year). In addition to the above, we also have a number of chromotripsis-like patterns (CTLP). CTLP is characterized by its relevance, immunity, and effectiveness. It is possible to analyze MLPA (Multiplex Ligation-dependent Probe Amplification) with high accuracy. It is possible to analyze MLPA with high accuracy. The structure of digital MLPA is different from that of MM. Special, good and bad patients should be compared, early diagnosis should be explored, and related factors should be explored. For those who have survived for more than 5 years and are good at it, the CTLP in the field is not detected. CDKN2A transmission loss frequency is low, and CN variation is high. The number of resolved samples increased, early detection and re-detection of MM patients were achieved, and molecular identification was achieved. This process requires the discovery of target molecules that induce instability and the development of future therapies.
