自己免疫性脳炎の患者は、一般に亜急性発症の精神症状、記銘障害、けいれん、意識障害などを呈する。近年、自己免疫性脳炎の診断マーカーとなる自己抗体が多数見いだされ、それぞれの臨床的特徴と抗体種との関連が徐々に明らかになっている。しかしながら、新たな抗体の発見が相次ぐ状況にあり、報告された多数の抗体を、疑わしい症例すべてについて検査することは困難であり、自己免疫性脳炎の全体像を把握できていない。申請者は、報告数の多い抗体を可能な限り検出するシステムの構築を進めており、中枢神経の興奮性・抑制性シナプス受容体を標的とするN-methyl-D-aspartate receptor ：NMDAR、α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic receptor:AMPAR, γ-aminobutyric acid type B/A receptor:GABAB/AR, leucine-rich glioma inactivated 1:LGI1, contactine-associated protein-like 2:CASPR2, glycine receptor:GlyR, DPPX, GluK2, GluDに加え、MOG, APQ4 に対する自己抗体を安定的に検出するcell-based assay法を確立しており、各施設からの診断依頼に対応している。これまでの集計では、最も検出数が多いのは従来通りNMDAR抗体陽性例であるが、近年、脳炎としての特徴的な症候を呈さずに慢性経過の認知症で推移する例の認知度が高まっており、今回の検討でも、認知症の診断の下に加療されていた症例が複数例見られた。特に、近年、急性脳炎を疑う症候が乏しい精神病、認知症、てんかんの中に、脳炎関連自己抗体が検出され、免疫療法が奏功する例の認知度が高まっており、抗体検査に至る例も増えている。今後はさらに多数例を集積して、自己免疫性脳炎病態の広がりを明らかにする予定で引き続き症例の集積に務めている。

In recent years, the symptoms characteristic of chronic dementia have been shown, and the cognitive degree of patients with chronic dementia has been high. In recent years, the diagnosis of cognitive disorders has been discussed, and the treatment of cognitive disorders has been increased. In particular, in recent years, acute inflammation suspected symptoms are lacking, mental illness, cognitive disorders, drug abuse, inflammation related to their own antibody detection, immunotherapy has been effective, the case recognition is high, antibody detection to the case increased. In the future, most cases will be collected, and the pathological changes of autoimmune diseases will be clearly defined.
