HLA半合致移植におけるHLAハプロタイプは、患者とドナーが共有するもの（共有HLA）、ドナーのみがもち患者がもたないもの（ドナー固有HLA）、患者のみがもちドナーがもたないもの（患者固有HLA）の３つが存在し、それぞれに拘束性のT細胞の存在が考えられる。従来、HLA半合致移植後の生体防御は共有HLA拘束性T細胞が担うと考えられてきた。これは日本人の1%強に存在するHLAホモ（２本のHLAハプロタイプがたまたま同じ）の人でも特段の免疫不全を呈さないことから、HLAハプロタイプは１本あれば事足りるはずという理屈に基づいている。では本当に共有HLA拘束性T細胞は移植後いつも存在するのか、あるいは患者固有HLAやドナー固有HLA拘束性T細胞はどうかを実際に見てみたいと考えた。そこで本研究ではCMV特異的T細胞を例に、HLA半合致移植の枠を超え、HLA不適合移植後の血液検体において、HLA-A*24:02およびHLA-A*02:01拘束性CMV特異的テトラマーを用いて、その検出を試みた。これまでにHLA半合致移植２０例、HLAフルアロ移植１２例、夫婦間移植３例、バンク移植１例、臍帯血移植１例から、のべ６４検体についてアッセイを行った。この時、共有HLA、ドナー固有HLA、患者固有HLAがたまたまHLA-A*24:02または02:01に該当しないとアッセイできないことに注意する。さてその結果、想定された通り、共有HLA拘束性T細胞は多くの例で検出された。ドナー固有HLA拘束性T細胞は時に検出され、患者固有HLA拘束性T細胞は１例でのみ検出された。
