本研究は、重症急性循環不全に対する体外式膜型人工肺(ECMO)使用時の左室後負荷軽減(LV unloading)方法として、経皮的左室補助装置IMPELLA(インペラ)の効果を検証するものである。具体的な方法として、ブタ の急性心筋梗塞 + V-A ECMOモデルで、2種のLV unloading効果の比較検討を行うことを目指している。同モデルにおいて、冠動脈左前下行枝(LAD)結紮を行い、その後のデータ追跡を行う。最終的には、ECMO単独、 ECMO + surgical LV venting、ECMO + IMPELLA の3 群比較を行い、循環補助中の血行動態、循環補助離脱後急性期の心機能、循環補助離脱後慢性期の心機能・心筋リモデリングを検証することが本研究の目的である。ECMO単独、 ECMO + surgical LV ventingの２群について、実験をすすめており、surgical LV ventingの有用性については確認できている。今後は、ECMO + IMPELLA 群のモデル作成と比較に取り組む予定である。
