突然の心停止は先進国において主要な死因である。蘇生法の進歩にも関わらず、心停止の誤の死亡率は依然として高いままであり、仮に生存した場合にも多くの患者には障害が残る。一部の患者には記憶障害、注意障害、注意機能障害などの高次機能障害が残ることが知られているが、蘇生後高次脳機能障害の動物モデルは存在せず基礎研究も進んでいない現状がある。我々はマウスを用いて心停止・心肺蘇生モデルを作り、自己心拍再開後に一酸化窒素（NO）を吸入することにより脳浮腫が軽減することを発見した。また、予備実験の結果から蘇生後のマウスでは記憶が障害されていることが明らかになった。以上のことより、本研究の目的はNO吸入による蘇生後の記憶障害に対する予防効果を検証する事である。これまでに蘇生後の記憶障害モデルは存在せず、短時間の心停止によるマウス蘇生後記憶障害モデルを確立できれば、この後の基礎研究の発展に貢献できると考えられる。さらにNO吸入により記憶障害の改善効果が認められれば、新しい治療戦略として臨床応用に繋げることが可能である。一昨年は手術手技の獲得を目標としたが、昨年度はマウス蘇生後記憶障害モデルを安定して作製することを目指した。手術手技（挿管、動脈/静脈カテーテル確保）の安定、体温保持、麻酔開始から心停止までの時間および、心停止から蘇生での時間の均一化などに努めモデルとしての均一化を図った。一部のマウスは不潔手技による敗血症で死亡したが、概ね安定した心停止・心肺蘇生モデルの作製を行えるようになった。今年度は心停止・心肺蘇生モデルに対してMorris water maze testにて記憶障害の評価を行い、モデル間での意識障害の程度の定量化および実際にNO吸入を行い効果の評価を行う。

Some patients have memory impairment, attention impairment, attention function impairment, and high-order functional impairment. The heart stops, the heart stops. The result of the preparation is that the memory is damaged. The purpose of this study is to investigate the effects of NO inhalation on postnatal memory impairment. This is the first time that the memory impairment has been established, and the contribution to the development of basic research has been made. The results of NO inhalation and memory impairment improvement are recognized and new treatments are available for clinical use. Last year, we achieved the goal of acquiring surgical skills, and last year, we achieved the goal of stabilizing the memory impairment of the patient after she was born. Stabilization of surgical technique (vascular, arterial/venous system), temperature maintenance, time of anesthesia onset, cardiac arrest, and time of cardiac arrest. A part of it is dirty hands, blood poisoning, death, heart failure, cardiopulmonary resuscitation, and operation. This year, the Morris water maze test was used to evaluate the memory impairment, quantify the degree of mental impairment, and evaluate the effect of NO inhalation.
