项目摘要

造血幹細胞移植を受ける小児がん患者について口腔粘膜状態の経時的な観察および口腔細菌叢の変化について検討した。患者の口腔内より移植前、移植1か月、3か月、6か月後に歯垢と唾液をサンプルとして採取した。採取したサンプルを、ミュータンスレンサ球菌数の測定にはバシトラシン含有 Mitis salivarius 寒天培地を、乳酸桿菌数の測定には Lactobacillus Selective 寒天培地に播種し、菌の分離を行った。また、菌種の特定のために、分離した菌から通法に従って細菌DNAを抽出し、得られたDNAをテンプレートとし、各菌種に特異的なプライマーを用いたPCR法にて、検体中に含まれる代表的な6種の口腔レンサ球菌と10種の代表的な歯周病細菌を同定した。研究には現在9名が参加し、年齢は1歳1か月より11歳3か月までであり平均年齢は6歳3か月であった。疾患名は再生不良性貧血が3名、急性リンパ性白血病が3名、急性骨髄性白血病が１名、骨髄単球性白血病が1名、Bリンパ芽球白血病が1名であった。口腔粘膜状態はWHOの口腔内有害事象スケール(Scale0～4。0が事象なし4が経口摂取不可)で記録し、移植が終了した8名のうち0が3名、1が1名、2が1名、3が3名であった。ミュータンスレンサ球菌は移植前に8名中4名に、移植後1か月では3名に認めた。また、乳酸桿菌は移植前には8名中１名に、移植後１か月では4名に認めた。さらにPCR法にての移植前のレンサ球菌種は8名中7名に認め、移植後１か月では4名にのみ認め、その１人あたりのレンサ球菌の種数の平均値は移植前では2.8種であり移植後１か月では0.9種であった。また歯周病菌の出現数は移植前では8名中4名に認めたが移植後１か月では2名に認めた。またその１人あたりの歯周病菌の種数の平均値は移植前で0.9種、移植後１か月では0.6種であった。
