「電子カルテ」システム内に登録しているクリニカルパス（以下パス）について保険診療の視点・DPCの視点より検討を行った。検討は「電子カルテ」内に登録されている約1900のうち、実際の使用頻度の高いパス300について実施した。その結果、大きく逸脱しているパスは存在せず、登録時のパス委員会での審査が適切に行われていることが立証できた。しかしながら、詳細に検討すると、パスに組み込まれている「管理料」・「指導料」のうち、「肺血栓塞栓予防管理料」における医師のリスク評価記載、弾性ストッキング装着の医師の実施指示、看護師の実施記録や、留置ドレーンの抜去時間の記載などに不備が存在し、パス内の指示は適切ながらも、その実施記録までの確認が必要なことが判明した。また、パスの使用率についても診療科によって大きく差があり、本院では、呼吸器外科、眼科、循環器内科、乳腺甲状腺内分泌外科においては入院患者において約９０％がパスを使用していた。DA（Doctor's assistant)による代行入力についても、外来診療では医師による承認が遅滞なくなされているのに対し、入院診療では一部遅滞もしくは未承認が存在し、今後の課題と考えられた。今年度、パスの検証に加えて「電子カルテ」の特性を生かし、医療者への情報公開の一環として「電子カルテ」診療データベースより抽出可能な「臨床指標」の取り纏めも実施し、これまで各部門等で独自に抽出していた指標を一元管理する体制を構築した。現在定期的に公開している指標は以下のとおりである。医用画像レポート未読率、医師・看護師退院要約作成率、DA代行入力承認率、看護師代行入力承認率、平均外来待ち時間、病名部位・詳細不明コード使用率、外来血液生化学検査項目実施数。これらの使用は「電子カルテ」第一面の「掲示板」にその結果を掲載し適切な保険診療への注意喚起となっている。
