本研究は、退院支援に関して在宅医療・介護職の視点から困難事例の要因を明らかにし、その要因改善のためのコミュニケーション・ツールの開発を目的としている。それを踏まえて、在宅医療・介護職からの課題を調査・整理し、病院医療職とのギャップに気づき、それを埋めるようなコミュニケーション・ツール（コミュニケーション・モデル等）の開発に取り組むことを目的としている。前年度はヒアリング調査の結果を基にアンケート調査の作成であったが、コロナ禍のためにヒアリング調査思ったように進まず、オンラインインタビューやメール、電話のやりとり等時間がかかったため予定がずれ込み、アンケート作成が令和4年度8月末までかかった。アンケート調査の実施は9月となったが、全国の介護に携わる職種(訪問看護師、介護士など)に調査をすることができ、503件の回収数を得た。
