项目摘要

わが国の健康診断のデータをデータベース化した健診データの利用可能性については、十分に検討されていない。そこで、健診データとレセプトデータがリンクした商用データベースを取得し検討することとした。健診データから脂質異常症の定義を満たす集団を、研究対象の集団として特定することが可能であった。脂質異常症の定義を満たす日をindex dateと定義し、index dateにおける合併症や併用薬に関する背景情報の取得のため、それ以前に過去6ヶ月間の記録のあるものに対象集団を制限する。アウトカムや合併症、併用薬を用いるコードを定義し、脂質異常症の集団における性・年齢や合併症や併用薬の使用などに関する観察開始時点における情報を集計することが出来た。また、健診データから、薬の非使用者の定義が可能であったので、脂質低下薬が非使用の集団を用いて、脂質低下薬の使用とのイベント発生との関連についての単純な粗解析を行うことができたので、今後は、さらに複数の交絡因子を調整した解析を行う予定である。イベントについては、レセプトデータに含まれる診断に加え、薬の使用によって定義が可能かについて検討を行い、具体的にアウトカムを定義することを予定していたため、今回のデータにおいても以前の検討結果を踏まえ、眼科に関する手術などを特定する前に、別の薬剤における血中濃度の測定に関する情報を取得し、検査や手術に関する情報について取得可能性について確認した。アウトカムについては、眼科疾患に関する診断に加え、手術について特定し、検討する予定である。レセプトデータを用いて血中濃度測定に関する検査の実施とアドヒアランスについても検討が可能であったので、これについては成果を論文として報告することが出来た。今後、脂質異常症の基準を満たす集団における眼科疾患との関係について最終的な解析を行う予定である。
