総合周産期母子医療センターにおけるMFICUの病床管理と看護管理者の役割

项目摘要

前年度から引き続き文献検討の結果を分析し、MFICUの病床管理に関する課題として以下4点があげられた。１）MFICUの１施設当たりの必要病床数の検討は十分されていない２）算定期間を超過した患者の収容により、母体胎児集中治療室管理料の算定が効率的に行われていない３）療養環境・生活環境としての病床選択がなされているか不明である４）看護管理者がMFICUの病床管理において果たしている役割が不明である。今年度は調査前の情報収集を行い、総合周産期母子医療センターの施設数が各都道府県HPと厚生労働省公表の一覧において一致していないことを把握した。そこで都道府県HPおよび同HPに掲載されている医療施設のHPを閲覧し、なぜ厚生労働省が公表する一覧と齟齬があるのか明らかにすることを試みた。その結果、総合周産期母子医療センターとして指定ではなく認定を受けている医療施設が複数存在し、認定施設は厚生労働省の総合周産期母子医療センター一覧には掲載されていなかったことが明らかになった。総合周産期母子医療センターの中には母体胎児集中治療室管理料を算定していない病床を有している施設があることも施設のHPより明らかになった。実際にMFICUの場には病床があっても母体胎児集中治療室管理料を算定しない病床がある理由は、算定要件を満たしていないため施設が届け出を提出していない可能性が考えられた。算定要件には、看護職員（看護師または助産師）の人員数、医師が24時間常駐する診療体制、1床あたりの広さ、機材や設備の整備などがある。全国周産期医療（MFICU)連絡協議会の調査では、産科医の常駐体制を維持することが困難であることが報告されていた。算定期間を超過後のMFICUへの患者収容により効率的に病床が活用されていないという課題以外に、複雑な要因がMFICUを取り巻く病床管理には影響していることが示唆された。
