本研究の目的は、ドセタキセルの療法を受けた乳がん患者の臨床的特徴を明らかにすることである。その内容は、(１)下肢浮腫の要因と特徴を明らかにするための後ろ向き観察研究、(２)明らかとなった要因を有する乳がん患者の下肢浮腫の経過の特徴を明らかにする前向き観察研究に分類される。(１)後ろ向き観察研究では、カルテ調査と質問紙調査を行った。乳がん患者のドセタキセル療法後に出現する下肢浮腫の要因はステージⅣの乳がんであることが明らかになった。(２)前向き観察研究では、診療録、問診より身体症状、視診、触診および画像所見から皮膚変化を観察した。画像所見はエコーを用いた。出現要因を有する対象者とそれ以外のステージの対象者を比較して個別に経時的に観察し記述した。自覚症状や圧痕が出現する前にエコー上、下肢浮腫の初期所見が出現することが明らかになった。またステージⅣの乳がん患者の下肢浮腫は重症化した。今後、ステージⅣの患者には、エコーを用いて初期所見が観察された時点からケアを開始し、投与終了後４カ月まで行うことを推奨する。
