【テーマ】難病患者の外来受診に関する現状の把握【目的】地域で生活する難病患者が外来受診の際に、医師や外来看護師、ソーシャルワーカーなどの専門職に対して期待する身体的・精神的・社会的ニーズを明らかにし、在宅療養生活の質向上に向けた、外来における看護支援内容を検討することである。【研究方法】対象者は茨城県に在住する難病患者とし、疾患群ごとの特徴を抽出するために、疾患群ごとに複数名の患者20名程度とする。対象者に対して面接調査を実施し、①疾患群の特徴を踏まえた外来ニーズの特徴と、②外来に期待する役割について聞き取りを行う。得られた内容は、質的研究に精通した専門家のスーパーバイズを受けながら分析し、外来における支援ニーズを抽出する。倫理的配慮として、茨城県立医療大学倫理委員会の承認を得る。また、難病患者の選定は、茨城県難病相談支援センター、茨城県難病連、それぞれの患者会等の代表者を通じて協力者を募集し、強制力が働かないように配慮すること、また協力者のプライバシーの確保と体調に配慮し、調査を実施する。【研究の意義】難病患者は、保健所や難病相談支援センター、訪問看護などの在宅医療支援により、在宅療養生活を送る上での困りごとなどを相談することができる。しかし、支援体制は十分ではない。一方で外来は、難病患者が定期的に受診する場であり、必ず看護師による支援を受ける機会がある。しかし実際には業務量の多さや難病看護に関する知識不足などにより、看護師は十分に支援が出来ていない現状がある。そこで、本研究を実施することにより難病患者のニーズが整理され、外来看護師が、難病患者に対する在宅療養支援を積極的に実施することができるものと考える。
