项目摘要

産後期の助産診断名および診断指標について、全国の産後ケア施設に所属する助産師を対象にその妥当性についての意見を聞くために調査を実施した。質問紙は、診断名、定義、診断指標について、夫々に「妥当である、どちらかといえば妥当である、どちらかといえば妥当でない、妥当でない」の4件法にて行った。調査期間は令和4年8月～令和4年11月末日迄で、77名の助産師から回答を得た。回答した助産師の勤務経験年数は平均18.0年、産後ケア施設での経験年数は10.3年であった。勤務先は病院42名(54.5%) 、診療所14名(18.2%) 、助産所13名(16.9%)であった。診断名、定義、診断指標について「妥当でない」と回答したものはみられなかった。「どちらかというと妥当でない」との回答があったのは、「産後の日数と月数の表現」、「排便のリズム」、「乳房の状態の定義」、「乳房の発熱」、「児の月齢と日齢」、「児の排便と熱感」、育児に向き合う姿勢の「児を愛称で呼んでいる」、「家族内行事を大切にしている」などであった。これらについては、頂いた意見を元に修正していきたい。産後ケア施設で実際に行っているケアは、乳房ケア72名(94.7%),個別指導65名(85.5%)が多く、休息のためが66名(86.8%)であった。育児支援として、体重測定、授乳、沐浴などを行っていた。これらの結果を踏まえて、産後期の診断名、定義、診断指標について加筆修正していく予定である。また、この結果の一部はICMにて報告する予定である。
