项目摘要

本研究で明らかにしようとしている産婦健診ケアモデルは、親が生活する地域コミュニティでの母子保健サービスに言及した看護支援も含まれると考えている。今年度、A県内での助産師交流会や周産期に関する会議に出席し、産後2週間健診と産後ケアサービスとの連携について、助産師、市町村保健師の実情に関する情報を得た。産後2週間健診でメンタルヘルスに課題のある褥婦は地域と連携したサービスにつなげているが、特に問題のない褥婦についてはその後の支援展開はあまりされていない実情がある。また、前研究課題からの分析を継続し、親が子どもの出産・育児に伴う新たな課題に取り組み適応するMasteryを促す２つのモデルが明らかになった（論文査読中）。知識・情報－Masteryモデルは育児に関する知識・情報をもつこと、その知識・情報によって育児の課題に対する効力感が得られること、また知識・情報を家族との生活の中で実践し、家族の統合的対処を促進することで乳児期の子どもをもつ親のMasteryが高められると考えられた。もう１つの家族：統合的対処－Masteryモデルでは、家族の統合的対処と家族員の精神的安定、育児ストレスが関連しあうこと、また精神的健康と家族の統合的対処によって、育児自己効力が促進されることにより、乳児期の子どもをもつ親のMasteryが高められると考えられた。この２つのモデルは母親、父親いずれにも適合するものであったが、知識・情報－Masteryモデルは父親で、家族：統合的対処－Masteryモデルは母親でMasteryを高めやすいと考えられた。これらの結果により、具体的な支援モデルを検討することが可能になると考える。
